In Search of the “Magic Bullet”

What’s New in Science and Technology No. 9, 2005 can't do anything about cancer? I am afraid, however, that the challenges faced by the developers of anti-cancer therapy are not up to the designers of weapons yet. It's the same as if they were offered to create a weapon capable of killing the terrorists who seized the building (and certainly all of them), without damaging the hostages located there.
About a hundred years ago, the chemist and biologist Paul Ehrlich introduced the concept of "magic bullet" into scientific jargon. He called them his dream - a drug that, when introduced into the patient's body, will itself find and kill the causative agent of the disease without harming the patient. Ehrlich immortalized his name by creating such "magic bullets" - sulfonamides, the first effective antibacterial drugs in the history of medicine (which include, in particular, the well-known streptocide). They and antibiotics, which appeared a little later, revolutionized the treatment of infections.
But the enemy they strike is a foreign organism. His enzymes, the chemical reactions that underlie his life, are very different from ours, and you can try to find a substance that is harmless to us, but deadly to him. In addition, it is not necessary to exterminate it to the last cell - it is important to kill the bulk, and our immune system will cope with the few survivors.
What do you want us to do when our own cells attack us? They need the same things for life that we need; whatever is poisonous to them will be poisonous to us. And any of them, having survived, can give rise to a new tumor, quickly nullifying all our efforts.
For a long time, the only treatment for cancer was surgical removal of the tumor. The accumulation of degenerated cells was usually cut out with a large supply of healthy tissue, often the entire affected organ (mammary gland, ovary, etc. ) was removed entirely. Nevertheless, such operations gave only a short respite from death: by the time the patient fell under the surgeon's knife, cells from the tumor, as a rule, had already begun to migrate. It was possible to find out where they had gone only after secondary tumors grew from them. Selective drugs were needed, and in order to create them, it was necessary to find something that distinguishes a cell that has broken off the coils from a healthy one.
One difference was striking: the cancer cell is continuously dividing. Meanwhile, from the first steps of radiobiology, it was known that it is dividing cells that are especially sensitive to radiation. Since the 50s of the last century, radiation therapy has firmly entered the arsenal of clinical oncology - exposure to the affected area of the body with electromagnetic radiation (hard X-rays or gamma rays) or a particle stream. In a sense, the homeopathic principle “treat like with like” was justified: radiation destroys rapidly dividing cells, because during copying their genetic apparatus becomes available to damaging agents. That is, the cell is vulnerable to radiation therapy for the same reason that it turned out to be vulnerable to mutations that led to malignant degeneration. But unlike homeopathy, radiation therapy uses doses that are much higher than the disease-causing ones, and the damage caused by them is incompatible with the life of the cell.
But radiation therapy is a local effect, cells that have migrated from the main tumor, as a rule, do not fall under it. In addition, almost all types of radiation are absorbed in the surface layers of tissues. Therefore, irradiation is effective mainly for the treatment of neoplasms in the skin and other integumentary tissues. There are, of course, special cases. For example, the hormone produced by the thyroid gland requires iodine, and its concentration in this organ is an order of magnitude higher than in any other tissue. Therefore, with thyroid cancer, the patient is fed a radioactive isotope of iodine, which itself concentrates in the right place and infects lawless cells there. But in most cases, radiation therapy of tumors of internal organs is not an easy task.
On the other hand, cytostatic chemicals are free from this restriction, the use of which in the clinic began almost simultaneously with radiation therapy - in the second half of the 40s. The first generation of cytostatics were simply poisons that selectively affected rapidly dividing cells. With such drugs, cancer chemotherapy began, which today has become the most extensive area of clinical oncology.
But not only cancer cells are constantly dividing in the body. Everyone has probably heard that "chemotherapy makes hair come out." That's right: cytostatics are detrimental to the cells of the hair follicles, which must constantly divide in order to ensure hair growth. Of course, when it comes to life and death, you can go without hair for several months. But what about the skin, the top layer of which is constantly dying and sloughing off and must also be continuously renewed? The hematopoietic tissue, the red bone marrow, is also under attack: human red blood cells live only about four months, and in order not to become smaller, precursor cells must divide as usual. However, erythrocytes are given to a person with a huge supply, but a sharp decrease in the number of leukocytes, which also suffer greatly from cytostatics and radiation, can lead to the death of the patient. Therefore, chemotherapy is usually given in cycles with breaks between them to allow the body to repair damaged tissues. (It is clear that in this case, the surviving tumor cells also get a respite.) In especially severe cases, when very high doses of cytostatics are required, the hematopoietic tissue dies completely, and after the drug is removed from the body, the patient has to implant it from the outside - either a donor or a pre-prepared own . One of the oncologists in the hearts compared chemotherapy with the treatment of syphilis used in the Middle Ages with mercury vapor, which almost equally poisoned both spirochetes and their victim. If we recall that the first of the cytostatics was a derivative of the chemical warfare agent - the famous mustard gas, this comparison does not seem so exaggerated.
“In recent years, tremendous changes have taken place in the drug treatment of malignant tumors,” says Vera Gorbunova, head of the Chemotherapy Department of the Russian Cancer Research Center of the Russian Academy of Medical Sciences. “Cytostatics have been replaced by so-called targeted drugs, i.e. targeted drugs created on the basis of new ideas about the process of carcinogenesis developed in previous decades by fundamental science.”
One of the groups of such drugs is monoclonal antibodies, immune proteins that can bind to specific molecules: receptors for growth factors, tyrosine kinases (enzymes that activate intracellular mechanisms of preparation for division), etc. Without directly killing the tumor cell, they fix it at one stage or another of the cell cycle, interrupting the chain of endless divisions and eventually bringing the rebellious cell to apoptosis. Of course, the course of treatment must be repeated from time to time, but a person can live for years and years. And in some cases, the goal of therapy is not the prolongation of the patient's life (even if indefinitely long), but a complete cure.
However, the arsenal of traditional therapy — drug and radiation therapy — in these years was also replenished with new drugs. More recently, the diagnosis of chorionepithelioma meant a death sentence - this tumor, which develops from the remnants of the placenta, i.e. in young women, rapidly metastasizes. But it turned out that she is very sensitive to the drug methotrexate - so much so that the current international standard for specialized oncology clinics includes the requirement of zero mortality from chorionepithelioma. The infamous small cell lung cancer has receded before a number of new drugs (which, by the way, include nitrosomethylurea, which itself is a strong carcinogen). Skin cancers, if treated before they metastasize, can be completely eradicated with modern radiation therapy. And on the wall of the office of the director of the Research Institute of Pediatric Oncology, there are numerous wedding portraits of former patients who came to him in the first years of their lives with terrible diagnoses.
Worth mentioning is the so-called neutron capture therapy, which gives very promising results. This relatively new technology differs from traditional gamma-ray therapy for the better - with it, a narrow beam of thermal "slow" neutrons extracted from the reactor is absorbed by atoms of rare earth elements, which at the same time begin to emit secondary radiation, which, in turn, destroys cancer cells. tumors (for this, a drug containing boron or gadolinium is injected into the tumor before irradiation). Neutrons act only on tumor cells saturated with these elements, so healthy cells practically do not suffer - secondary radiation extends only a few microns. In addition, one session is enough for neutron capture therapy.
And yet, in most cases, seemingly defeated, the tumor returns again and again. Moreover, drugs that once stopped its development, the second time, as a rule, do not work. “The main problem with a malignant tumor is that there are different cells. They have an unbalanced genome, and there are freaks for all occasions. And therefore, no matter what drug we use, there will always be a dozen resistant cells. They gradually multiply, and a tumor appears that is already resistant to this method of treatment,” says Gennady Belitsky, Doctor of Medical Sciences, Head of the Laboratory of Carcinogen Screening Methods at the Research Institute of Carcinogenesis of the Russian Cancer Research Center.
True, among the drugs of the new generation there is a group of drugs that still manage to bypass this vile feature of cancer cells. Back in 1971, the American physician Judah Falkman drew attention to the fact that the rapid growth of a tumor and the formation of metastases begin only after blood vessels grow inside the neoplasm. In the 80s, researchers managed to decipher the chemical signals by which tumors attract capillaries to themselves, and in the 90s, Folkman's employee Michael O "Reilly isolated and identified two substances that suppress this effect. Today, the number of angiostatics - drugs that block the growth of blood vessels , - is measured by many tens, and for some of them it has been proven that there is no decrease in effectiveness with repeated use. The fact is that these substances do not act on malignant cells, but on completely healthy vascular cells that have neither genetic instability nor rapid reproduction - features that allow cancer cells to escape from the action of drugs.Angiostatics have one more plus: in an adult healthy body, the need for laying new capillaries occurs only during pregnancy or regeneration of damaged tissue.In all other cases, the use of angiostatics can damage only tumors, which , having lost its blood supply, not only stops further growth, but it seems to shrink to a much more modest size.
Angiostatic agents are now widely used in the clinic, but it is already clear that they have not become the “magic bullet” that would solve the problem of cancer in general. The enemy of oncologists is too changeable and many-sided, and we owe all the victories over him not only to technologies and methods, but also to ordinary luck.
“Now all the successes in the treatment of tumors are due to the fact that each tumor has its own characteristics, playing on which, you can cut down one of their classes after another,” says Gennady Belitsky.
